How to claim insurance

Once you open a claim which is made simple with our help, you are usually covered up to $3000 for Physio treatment

At Holistic Physio Fitness, we have made it even easier for you to get your insurance claim up and running. We have taken extra lengths and done almost all the legwork (apart from filling out your forms), to make sure you can get the insurance coverage from Rugby NSW in a timely and easy fashion. What to do:

All claims need to be lodged with the Insurer and treatments completed within 12 months from date of injury. On the first page of the claim form you will note the comment that claims need to be lodged within 30 days of injury, please be advised that this is just an indication and your claim can still be lodged after this time.

Once you have sign off from their treating physician & club official (President, VP, Secretary or Treasurer) you should make copies of everything for yourself, then send through to the insurer, SLE [email protected] (1800 002 676)

SLE will issue claim number that will be the point of reference when forwarding supporting documentation or liaising with the Insurer. Please allow 5-10 business days for this turnaround.

Step 1:

Book in with us by calling 9999 6666 and saying what club you play for when booking so we can book you with the right therapist. Therapy can start straight away and the insurance company will reimburse you for those costs.

Step 2:

Download the ARU Claim Form There are multiple easy sections. One section that you fill out, one section that the club secretary or president fills out and the other section that a GP fills out. Note this must be filled out by a Doctor, not the Physiotherapist. We have a GP that can see you for no charge and fit you in asap. Please call us on 9999 6666 and ask for Andrew
